To ensure smooth operations, our office adheres to a 5-minute late policy for existing patients. If you anticipate being late, kindly give us a call to inform us. If your arrival extends beyond the 5-minute grace period, we may need to reschedule to prioritize on-time scheduled patients.

For your convenience, we encourage new patients to complete paperwork before the scheduled appointment. Access our new patient forms HERE. If online completion isn't feasible, you can fill out the paperwork in-office. If opting for in-office completion, kindly arrive 15 minutes before your scheduled appointment. Please note, if you arrive at the scheduled time without completing new patient paperwork, you may be asked to reschedule. Your cooperation ensures a streamlined and efficient experience for everyone.
